Our Primary Health Care Team provides primary health care, illness and disease prevention and health education services to immigrants and refugees of all ages who live in the City of Toronto. We are committed to providing culturally sensitive and language accessible health care to disadvantaged refugees and newcomers to Canada. Clients are referred to us from immigrant and refugee serving organizations and through our outreach work in recently established communities.

Primary Health Care Interdisciplinary Team

Our Interdisciplinary Team consists of:

  • Certified Diabetes Educators
  • Client and Family Support Workers
  • Community Health Nurses
  • Interpreters
  • Medical Secretaries
  • Nurse Practitioners
  • Physicians
  • Psychiatrist (through the Toronto Urban Health Alliance)
  • Registered Dietitians
  • Registered Practical Nurses
  • Social Workers


Access Alliance is often the first point of contact for newcomers within health services in Toronto.  We are structured to provide support and services to newcomers during this transitional period and effectively link them to the broader range health and settlement services.

Services provided by our Primary Health Care Team include:

  • Preventative health care – screening, teaching and education
  • Routine physical exams
  • Immunization for children and adults
  • Assessment and treatment of acute episodic illness
  • Ongoing management of chronic disease
  • Pre- and post-natal care and well-baby care
  • Triage service for urgent client problems
  • Therapeutic counselling for individuals and families
  • Mental Health services
  • After hours phone consultation
  • Information, advocacy, service referral and practical support

Most services are available in the clients’ language of choice, through our network of qualified interpreters.

At Access Alliance we know that the health of immigrants and refugees is greatly influenced by their experiences of migration, and settlement in a new country.
