Marianhill has 29 Continuing Complex Care beds; 26 beds are designated for patients requiring "slow stream" rehabilitation and patients with complex care needs. Three beds have been designated for palliative care.
To provide client focused care during a predetermined length of stay, to promote independence and optimal function in order to allow the client to return to their place in the community.
Marianhill’s interdisciplinary team works together with the client and their family to develop the client's plan of care.
This care plan establishes client centered treatment goals and treatment plans, and discharge planning. The care plan is evaluated by the team and client on a weekly basis. Clients/family participate in a care conference early in their stay to identify goals of care and to identify and implement required community supports upon discharge.
Health education primarily in the methods to assist in resuming normal activities and self-medication are important aspects of the treatment plan. The client and family are provided with information and education in these aspects of care.
Upon admission to the unit, the client participates in an interdisciplinary assessment process that incorporates physical, functional, psycho social and nutritional needs throughout the continuum of care towards discharge.
