UOHS is pleased to offer a travel clinic to help you stay healthy while you’re travelling so you can get the most out of your trip.

 

Your visit with a travel medicine specialist will include a health risk review of your travel itinerary, personalized travel health information, travel prescriptions and all required immunizations, including yellow fever. 

To help us serve you better please complete this and bring it to your visit.  In addition, bring your travel itinerary and your immunization booklet.
For more information on fees for consultations and immunizations at competitive rates, please call (613) 564-3950 and request a travel clinic appointment.
It is important to note that travel clinic services are not covered by student fees or provincial health plans so you will have to pay for your visit and immunizations at the time of your visit. For fees consult the.
