West Carleton Family Health Team - health portal

This portal will allow you to view some of the information stored in your Electronic Medical Record and also allow you to add information on a regular basis such as blood pressure and weight. Patients can access education on risk factors, including the associated health risks and life style changes they can implement in their daily lives to reduce these risks.

If you ARE NOT FASTING, it is suggested that you come after lunch to minimize wait times. Hours of operation: Monday to Friday (except posted holidays) 7:30 AM to 3:30 PM. Our Blood Lab is for the patients of the West Carleton Family Health Team only. We suggest you plan to come Tuesday to Thursday morning.

West Carleton Family Health Team - obstetrics

We provide care to patients who already have a family physician in the community as well as those who do not currently have a family physician. Some of the physicians of the West Carleton Family Health Team provide obstetrical care (care during pregnancy and delivery). Please contact our office -839-3271 to book a prenatal appointment and advise our staff who your current health provider is.

Preventative medical care: by using the best available evidence, we provide the information and resources for patients to maintain good health and prevent disease. Maternity Care and Obstetrics has been provided by the physicians for more than 25 years, with deliveries being done at the Queensway Carleton Hospital.
