Incident Analysis

When an organization experiences a serious incident the people involved are often traumatized and the entire organization is affected. Fatalities and serious incidents will have long lasting organizational impact. Many organizations experience dozens of smaller incidents near misses prior to a serious incident taking place. Many organizations do not examine these incidents or conduct any type of trends analysis on the incidents taking place. Conducting a detailed analysis of your investigative system and the incidents that have occurred will help prepare the organization and prevent recurrence of the incident.

Often organizations complete many near miss incident or actual incident reports including recommendations. The issue is that many of these reports and written recommendations do not always address root causes or the systemic changes necessary to prevent recurrence of the incident. Efficient incident analysis provides trends analysis and an analysis of the quality of the existing reports. Incident analysis will assist in determining causal chains beginning with the examination of the actual information collected at the start of the investigation including witness statements, documents reviewed observations made. Any steps missed in the original investigation will be identified and recommendations to improve these steps will be written.

A gap analysis report will be generated recommending changes to the organization that will allow them to improve their in house investigation protocols. The scope of this service could be one incident or it could be many incidents to give an organizational improvement scope to the investigative process.

We can act as a support liaison between you the owner and contractors providing analysis on their incident reporting and investigation systems to ensure all action items they are recommending are effective and the recommendations are implemented. Contractors who have experienced an incident are required to put the appropriate action plans in place to prevent recurrence.
