Mom Care

The Mom Care Program is a custom made solution designed to deal with the difficulties patients have finding physicians who will do obstetrical care, and the need for physicians to share the heavy burden of being on call 24 hours a day for obstetrics. Patients and their new borns may see the Mom Care physician for the six week check up, or return to their family physician for this and subsequent appointments.

Chronic Disease Management

Teams of medical professionals are being made available in physicians offices or at local hospitals to assist patients living with chronic disease to optimize their health and well being. Nurses working in the physician's offices work closely with patients to help inform them about their chronic condition, and to teach patients ways to manage their own health.

Multidisciplinary Team

Multidisciplinary teams were created to increse access to health care for patients who found it difficult to meet with a physician, whether it be transportation issues or language barriers. These professionals work together to provide comprehensive care of basic health needs with an emphasis on these priority areas.

Office Based Nursing

They contribute to the delivery of comprehensive care following best practices with the patient's goals in mind. Nurses who work in the family physician's office provide in-clinic support with a focus on the following areas. The patient-physician relationship is the most important relationship in family practice. The office based nurses enhance and compliment that relationship.
