Patients often find they can’t get all their questions answered during a brief visit with their physician. This is especially true for patients who are trying to develop new lifestyle and behavioural habits, such as losing weight, managing stress, or making other changes in their personal lives.

The Behavioural Health Consultant (BHC) team of the Calgary West Central PCN can help by working alongside physicians to give patients the extra support they need. The BHCs can help identify barriers to change, offer motivational support, or direct patients to services and supports available in the community. It’s all part of a program that gives patients a little extra time with the many health professionals at the CWC PCN.

After Hours Clinic and On Call Program

Nurses ensure that patients who need to be seen within four hours on weekdays, or within 24 hours on weekends and holidays, are either connected with the on-call physician, seen in the after-hours clinic, or directed to the nearest emergency room. Cuts, sprains, fevers, insect bites, mild asthma attacks … a lot of pressing health concerns and minor emergencies like these end up in the emergency room just because patients have nowhere else to go.
